Canada scores French in CLB (NCLC) levels, not CEFR. Many Express Entry candidates aim for CLB 7 in every skill, roughly B2; check the requirement for your programme.

Yes, in practice. The results certificate is valid for two years, and an application needs one that is still valid.
There is no pass or fail to repeat: you take the test again for a new result. A minimum waiting time between two sittings may apply; check the current rule with your centre.
Your centre gives you a results certificate, usually within a few weeks, with a score for each skill; IRCC converts the scores to CLB (NCLC) levels.
